(this form must be filled out to work with children)

(this form must be filled out by parents of children who wish to participate in church activities)

Incident Report
(this report is for Youth directors/leaders only)

Application for Minors to work with Children
(this form is for youth who wish to work with children)

Permission for Background Check
(all persons who wish to work as a volunteer with children and youth must fill out this form)

Suspected Child Abuse Form
(use this form if you suspect child abuse has or is occuring to report it)

Youth Participation, Medical Release and Covenant Form
(this form is for the youth who wish to participate in church activities)
